Experienced a mental health crisis in 2015? then please help

In the North East Lincolnshire region, a large number of local health professionals and organisations (known as ‘The Concordat’) have been working together since December 2014 to ensure that anyone experiencing a mental health crisis has access to the help and support they need, twenty-four hours a day, seven days a week. With the Concordat now a year old, it is therefore an appropriate time to assess its progress, from the point of view of the people who have had to use local mental health crisis services in 2015.

We would be grateful therefore if you could tell us about your experiences, by completing a questionnaire which has been formulated by local social enterprise, Open Door. This questionnaire can either be completed online, by visiting https://www.surveymonkey.co.uk/r/OpenDoorSurvey2016

or you can contact Jamie McDonald at jamie.mcdonald1@nhs.net  or 01472 722000who will send you a printed copy in the post.  Alternatively, you can visit Open Door in Albion Street, Grimsby, where their staff members will be happy to help you to complete the questionnaire and answer any questions you may have.

The closing date for completing the forms is 31.3.2016 and we encourage as many people as possible to make their views known, as the results will be presented to the Concordat, who will look to use this information to improve the quality of crisis mental health care in the region.
