Kylie has been shortlisted in the NHS Quality Champion / Innovator of the Year 2013 category for her work over the past 8 years improving services in North East Lincolnshire for people suffering from chronic respiratory diseases, such as Chronic Bronchitis and Emphysema (COPD) and people at risk of falls.
The Hope Specialist Service is a ground-breaking collaboration between a specialist multi-disciplinary team of falls and respiratory health professionals and its team of more than 90 volunteers, aged between 8 and 88, many of whom first encountered Hope Street as patients. The collaboration has enhanced and brought to life an ever changing and dynamic service that demonstrates outstanding clinical outcomes for patients, whilst also irreversibly touching the lives of the health professionals and volunteers who are integral to its delivery.
This nomination for Kylie comes at an exciting time as the Hope Specialist Service recently won a National Association of Primary Care award for Joint Working Initiative of the Year and has also been shortlisted for a prestigious Health Service Journal (HSJ) Award in two categories – Primary Care Innovation and Primary and Community Service Re-design.
