Care Plus Group provides a 24 hour telephone triage service.
This service is staffed by local nurses, with local knowledge of all the service available in North East Lincolnshire and able to give health advice to patients or access to GP out-of-hours appointments.
Unlike calling your GP [practice, our triage service provides a responsive service so you won’t have to wait for a call back but will get through to a nurse who can offer advice when you need it.
Our nurses have a range of experiences and are able to give a wide spectrum of advice.
The service also uses a clinical decision software programme, as an aid to triaging patients; this software is evidence based and uses the latest information.
Our Telephone Triage Nurses sit in the same building as a Community Urgent Care Team, making it easy for them to communicate with Community Urgent Care Team (formally Rapid Response) and fast-track a patient in need of this service and ensure they receive immediate care.
