The Community Inpatient Unit (Previously Cambridge Park) provides both short-term rehabilitation and reablement services to those patients who need additional support after leaving the hospital and before being discharged home, and also provides care for those needing it to prevent them from being admitted to the hospital.
We are a residential unit providing a 24-hour-a-day, seven-day-per-week service where the individual is recognised as the expert in their journey, and we work with them to achieve their rehabilitation goals.
Staffed by a multi-disciplinary team including experienced support workers, Occupational therapists, Physiotherapists, Social Workers, Nurses, therapy and social care support staff who are trained in techniques to support and encourage people to regain their skills and mobility. We also liaise with the individual’s own GP, District Nurse, the hospital, Adult Social Care and family in order to ensure a seamless service.
The aim of the service is to get you back to where you want to be following a period of illness or crisis. We want to help people to maintain their independence and stay in the community wherever possible, and avoid extended hospital stays or unnecessary care placements.
