Our Community Nursing Team are also able to care and support those people who are living with long-term conditions.
A long term condition (LTC) is defined as “those conditions that cannot, at present, be cured, but can be controlled by medication and other therapies”.
The life of a person with a long term condition is forever altered – there is no return to ‘normal’.
The Complex Case Team provide nursing care and support related to long term condition management to people and their families and carers in their home.
Referral may be from the GP practice, specialist nurses, secondary care, Focus or community nursing to name a few.
The Complex case team work in partnership with patients and their families/carers and the wider multi-disciplinary team to provide person-centred, coordinated care which enables people to make informed decisions that are right for them. The aim to help empower patients to self-care for their long term condition’s in partnership with health and care professionals.
This approach utilises personal care and support planning to reduce exacerbations, improve care and reduce hospital admissions, thereby improving quality of life.
