The Care Plus Group Stroke Service works with the Enhanced Community Stroke Team and Stroke Association to enable people to become as independent as possible following a stroke and enable them, wherever possible, to access facilities in their local community.
There are five local stroke clubs who are either affiliated to the Stroke Association or supported by Care Plus Group.
The Community Stroke Group on a Friday at Cromwell Road Health and Well-being Centre is a collaboration between Care Plus and community therapists. All the clients who attend have specific goals they need to work on to become more independent.
For further information regarding what is available in the community for people following a stroke please contact Amanda Young, Stroke Service Coordinator for North East Lincolnshire
